Medical Case

Chronic Heart Failure (CHF) is one of the most common causes of deaths worldwide.

However still, it quite often gets primarily misdiagnosed. Depending on research, this applies to 16-68% of cases. Most often CHF is being Initially mistaken for Chronic Obstructive Pulmonary Disease (COPD), and even if identified determining its type (HFrEF or HFpEF) is also a difficulty.

A conclusive diagnosis requires many tests and visits to specialists, which makes it possible to obtain one only after several weeks. Until then, the patient can only rely on possible symptomatic treatment.

How Mudisocard works

We have developed a device that records the following three signals from the human body: 3D ECG, 3D impedance and chest auscultation. These measurements are made using 7 electrode tips placed on the patient's body. Several minutes of signals coming from the chest are simultaneously recorded with a common time base. The recording is transmitted telemedically to the platform, where an algorithm compares each heartbeat with previous ones, analyzing their dynamic patterns. The result of this process makes it possible to immediately distinguish HF from COPD, and thus diagnose HF even for a primary care physician.

Benefits

Benefits

  • A doctor doesn't have to guess when referring a patient to a specialist
  • No costs wasted and no time lost for misdirected tests
  • Immediate recognition of COPD vs. CHF and of HFrEF vs. HFpEF
  • Start of treatment possible in one visit right away after the test

New knowledge thanks to 3D

The advantage of Mudisocard over conventional devices recording ECG signals in the Einthoven, Goldberger, and Wilson systems is a complete recording of the electrical function of the heart. In example, measuring signals in 3D gives the ability to unequivocally determine whether a P-wave is present in the acquired signal.

Upon request, Mudisocard can transform the three-dimensional ECG signal into a format corresponding to standard reference systems. This will allow reading study records in the form of traditional charts and applying the medical personnel's existing knowledge to their evaluation.

The possibility to choose different planes of ECG signal projection provides a unique opportunity for original research not accessible through traditional methods.

Simultaneously with the ECG, Mudisocard also measures tissue impedance in 3D. This measurement takes place in the complex domain – studying both the magnitude and phase significantly enhances the accuracy of the measured impedance.

No other device on the market provides this possibility. While there are cardiac output (CO) impedance monitors available, they do do not provide module and phase of the bioimpedance signal in 3D domain.

Simultaneous signal measurement by Mudisocard is a significant advantage of the method developed by EFM. The integration of 3D cardiography, 3D impedance, and infrasound from the stethoscope, precisely synchronized with each other, provides access to previously inaccessible information hidden within the interdependencies of these signals.

Synchronous analysis of signals measured simultaneously in different channels allows the extraction of potentially clinically relevant information from the content contained in their correlations.

Both images obtained by Mudisocard show signals representing the medical state of two hearts (3D heartbeats' record of a healthy person and of a patient suffering from HF). Analysed by The Algorithms, these signals provide doctors with indications for plausible medical conclusions.

Misdiagnosis of heart failure:
a systematic review of the literature.

"Misdiagnosis of cardiovascular conditions can lead to serious patient harm. This systematic review explored misdiagnosis of heart failure. Based on 10 included studies, the rate of heart failure misdiagnosis ranged from 16.1% (in an inpatient setting) to 68.5% (when general practitioners referred patients to specialists). Included studies found that heart failure is frequently misdiagnosed as chronic obstructive pulmonary disease (COPD)."

Heart Disease Misdiagnosis and Underdiagnosis – An Ongoing Crisis

"The frequent misdiagnosis and underdiagnosis of heart disease is an ongoing healthcare crisis in the United States. An underdiagnosis is a consistent failure to recognize or accurately diagnose a disease or condition in a large population of people, such as women or minorities. A misdiagnosis is a delayed or incorrect diagnosis of an illness."

Prevalence, outcomes and costs of a contemporary, multinational population with heart failure

"We estimate that 1–2% of the contemporary adult population has HF. These individuals are at significant risk of adverse outcomes and associated costs, predominantly driven by hospitalisations for HF or CKD. There is considerable public health potential in understanding the contemporary burden of HF and the importance of

optimising its management."
